Vacation Request Form

At The Job Shoppe, we prioritize your wellness and understand the importance of taking time off to recharge and handle personal matters. Whether it's for a well-deserved vacation, attending a medical appointment, or managing personal responsibilities, taking time off is essential for maintaining a healthy work-life balance.

  • Eligibility: After one (1) year of employment, associates are eligible for two (2) weeks of vacation
  • Submission: Submit the Vacation Request Form at least two (2) weeks in advance
  • Response Time: Approval or denial typically within 24 business hours via email

Note: Vacation requests must be submitted through the official form only; phone calls, emails, or verbal requests will not be accepted. A separate form is required for each vacation day requested.
