Extended Work Leave Request Form
Under the Employment Standards Act, associates are entitled to a variety of work leaves based on specific criteria.
- Types of Leaves: Information about work leaves in Ontario can be found here in the Employment Standards Act, under the leaves of absence section.
- Request Process: For planned leaves, submitting your request at least two weeks in advance is advisable. For emergency leaves, notify your supervisor as soon as possible.
- Leaves of Absence are Unpaid: Associates may be eligible for Employment Insurance (EI) benefits. More information about benefits can be found here.
- Response Time: Approval or denial typically within 24-48 business hours via email. The Job Shoppe will not accept any other request methods, including phone calls, separate emails or verbal communications.

Sick Days vs. Leaves of Absence
It is important to note that leaves of absence are not intended to be used for sick days. Associates who are unable to attend their scheduled shift due to illness must follow the call-in procedure specific to their assignment. Leaves are reserved for extended absences and require approval prior to the start of the leave period.
Calling In Until Leave Approval
Until a leave request is officially approved, associates are required to follow the call-out procedure for every shift missed. Failure to do so may result in attendance infractions.
